The Meaningful Opportunities for Successful Transitions (MOST) program

On June 30th, 2008, JFGH opens a unique program of services and support for young adults with disabilities with the goal of helping them make a successful transition from school to a placement at an established vocational or day habilitation program.

The MOST program will:

  • Help fill a pressing need in the community by providing services to individuals who need, but have not found, a placement in an appropriate day program after aging out of educational entitlements.


  • Offer an individualized program of activities and training based on each participant’s assessment and goals.


  • Serve as a portal for individuals and their families, introducing them to a broad range of community resources which will, in most cases, include an introduction to and collaboration with outside vocational or day habilitation programs. This collaboration will help individuals identify and become familiar with programs they might wish to enroll in, and allow outside service providers to likewise become more familiar with an individual’s capabilities.


  • The MOST program will operate from the newly renovated JFGH headquarters in Rockville, MD which includes a large activity center, laundry facilities, accessible bathrooms, and a kitchen. The centrally located facility is close to public transportation, shopping centers, and recreational areas. Assistive technology will be available, as needed, to support individuals.
