If you or someone you know is interested in receiving residential services from JFGH, the first step would be to contact the main office.

During the initial conversation any questions you have about JFGH and our services will be answered and we will obtain basic information from you regarding the individual in need of services. Following this conversation you will recieve a packet of information including an application for services, a JFGH handbook, and other JFGH literature.

Once you have completed the application, mail it back along with the $75 application fee. It would be helpful to include other important information such as a copy of recent medical exams, psychological testing, individual plans, and DDA funding correspondences.

Your application will then be processed and a confirmation letter will be sent that lets you know that JFGH has received the application and the individual needing services has been placed on the Active Waiting List.

When an opening becomes available that meets the need of the individual on the Active Waiting List, you will be notified  and will be invited to meet for an interview. Several individuals and their families will be interviewed for one vacancy, and will then be presented to the Admissions, Review and Discharge (ARD) Committee who will decide which individual will be offered placement.

The length of time that an individual waits for placement varies. Preference is given to those individuals who have been waiting the longest; however, it is more crucial that the vacancy be one that meets the needs of the individual. JFGH sends out an update form every other year to ensure that the individual files are current.

There is a $95 annual fee for all waiting list applicants. This fee helps support many of the programs and presentations JFGH offers to Waiting List families and advocates throughout the year.
